One big note you need to know! MacCready's Kill Shot perk is glitches, gives you a 2000% headshot bonus chance instead of 20%. If nothing works, well then, you'll need to load a previous save even if it's 4 hours before, if you really want to complete that quest. So what I'm saying is try everything, maybe even dismiss him and take him back, leave the area and come back in.
